I understand that by enrolling my pet(s) in pet sitting, boarding, SniffSpot, or daycare services at The Pet Paradise:

1. I assume all risks of injury, illness, or harm to my pet during the duration of its stay or care.

2. I acknowledge that unforeseen issues may arise, including bites, fights, scratches, or other injuries. I understand that The Pet Paradise will take all reasonable precautions to ensure my pet's safety but cannot guarantee a risk-free environment.

3. I agree that I will not hold The Pet Paradise or Jaime Dawn Cantley liable for any injuries or damages my pet may experience, including but not limited to illness, escape, injury, or death.

4. I understand that in the event of a medical emergency, The Pet Paradise will contact my veterinarian or an emergency veterinary clinic if necessary. I authorize reasonable medical treatment for my pet. I agree to be responsible for all associated veterinary costs.

5. If my pet is found to have fleas or ticks, I agree to cover any necessary treatment expenses before or after my pet's stay.

6. I acknowledge that Pet Paradise will provide boarding, pet sitting, and care services for my pet(s).

7. I certify that my pet is in good health and has no known illnesses or conditions that could jeopardize the safety of other animals or staff. I confirm that my pet is up-to-date on all required vaccinations, including rabies, distemper, and bordetella (for dogs). I will provide proof of vaccinations before my pet's stay.

8. I acknowledge that Pet Paradise is not liable for any illness contracted by my pet during its stay, including kennel cough or other communicable conditions.

9. Emergency Care Authorization I authorize Pet Paradise to seek emergency veterinary care if deemed necessary. I agree to cover all associated veterinary costs, including transportation, treatment, and medication.

10. I confirm that my pet has no history of aggressive behavior towards other animals or people. I assume responsibility for any damage or injury caused by my pet to property, staff, or other animals.

11. I understand that Pet Paradise reserves the right to refuse service or remove my pet if it poses a danger to others.

12. I understand that personal items (e.g., bedding, toys) brought for my pet are not the responsibility of Pet Paradise and may be damaged or lost.

13. I understand that my pet's participation in boarding or pet-sitting services involves inherent risks, including injury, stress, or illness, and I waive all claims against Pet Paradise for any such occurrences.

14. I agree that Pet Paradise will not be held responsible for injury, escape, illness, or death of my pet unless caused by gross negligence.

15 I agree to Pet Paradise's cancellation policy, including any fees for late cancellations or no-shows. Deposits are non-refundable unless otherwise stated in the cancellation policy.

16. I give consent for photos or videos of my pet to betaken and used for marketing or social media purposes by Pet Paradise.

17. I have read and understand the terms of this Consent and Waiver Agreement. By signing, I agree to the above terms and release Pet Paradise and its staff from liability for issues that arise during the care of my pet.
